• Low case load to achieve a full-time status (4 admissions per week)


• High Pay for work: $66,500 annual salary+ health insurance.


• You get paid for case management time.

Duties Include:

• Case Management Model of Care where you will deliver care to acute and chronic homebound patients in a patient centered care model.


• You will provide a variety of clinical skills such as IV therapy, wound care, and lab draws and psychosocial skills of teaching and management of disease process.


• The case management of patients in the home setting and includes OASIS data collection assessments at various time points. Such as start of care , post hospital resumption of care , recertification and the safe discharge of the patient to the community or to other health care settings.Requirements: * At least one year experience as professional nurse * The ability to make sound professional clinical judgments. * A car to be used for work, current driver’s license, and proof of auto insurance. * Willing to travel within the Tri-County (Wayne, Oakland, and Washtenaw) area to visit homebound patients.

• * Ability to function in any home situation regardless of age, race, creed, color, sex, disability , or financial condition of the patient.


• * Proficient clinical skills.


• * Competency in performing home care comprehensive assessments.


• * A current State professional Nursing License.
