• Review patient’s medical history, health assessment, medication


• Complete health assessment (over the phone), medication reconciliation


• Follow-up with the patient on a monthly basis

• College (Preferred)


• Licensed Practical Nurse


• Good Communication Skills


• Self-management


• Proficient Writing Skills


• 1 years of experience working in a medical practice


• EHR/EMR experience
