Job description:
Provides Supportive Services to Consumers with Developmental Disabilities, and works with Department of Developmental Services (DDS) Case Manager in the development, implementation and supervision of Consumer’s Individual Plan.

Job requirements:
Applicants must have a valid Connecticut driver’s license, fully insured own transportation, at a minimum a High School Diploma or General Equivalency Diploma and a minimum of two years of experience working with Developmentally Disabled Individuals and familiar with Dept. of Developmental Services regulations. Fluent in Spanish preferred.

Duties:

• Supervision of Direct Care Staff


• Ensures Direct Care Staff comply with Policies and Procedures


• Reviews Progress Notes for Accuracy and Content


• Reviews Direct Care Staff timesheets and schedules.


• Coordinates and/or conducts training with staff in accordance with Policies and Procedures


• Conducts random spot checks on Direct Care Staff and Consumer


• Coordinates medical, mental health, dental, educational, employment opportunities, vocational and educational services for consumer


• Conducts bi-weekly meetings/training with staff


• Reports all consumer and staff related information timely to Executive Director


• Reports incidents to Executive Director upon discovery


• Provides monthly and yearly reports to Executive Director


• Attends consumer progress meetings with the CT Dept. of Developmental Services


• Maintains open line of communication between Hispanic Coalition and the CT Dept. of Developmental Services


• Prepares monthly Vendor Authorization for billing


• Participates in Direct Care Staff hiring process


• Maintains and updates Employee Personnel files


• Provides New Employee Orientation


• Completes Employee Evaluations in accordance with Policies and Procedures


• Maintains a valid driver’s license and auto insurance


• Performs related duties as assigned by Executive Director

Required experience:

• Developmental Services Experience: 2 years

Required license or certification:

• Drivers License, PMT Trained, First Aid/CPR

Required education:

• High school or equivalent
