Enable Healthcare is looking for an individual with great patient interaction skills to contact patients on a monthly basis and speak to them about their health and manage their chronic conditions.

Responsibilities

• Review patient’s medical history, health assessment, medication


• Complete health assessment (over the phone), medication reconciliation


• Follow-up with the patient on a monthly basis

• High School Diploma


• College (Preferred)


• Medical Assistant Certification National


• Good Communication Skills


• Self-management


• Proficient Writing Skills


• 2-3 years of experience working in a medical practice


• EHR/EMR experience
